The Medical Discharge Planner for the Norfolk County Sheriffs Office is responsible for ensuring continuity of care, securing health insurance coverage, and coordinating post-release medical and behavioral health services for incarcerated individuals. This position oversees the planning, development, and facilitation of discharge-related services to ensure that individuals leaving custody are properly insured and connected to appropriate healthcare and community-based support services. The goal is to support successful reintegration and improve reentry outcomes. The Medical Discharge Planner shall adhere to the Department's Chain of Command policy and operate within the Administration Division under the direction of the Assistant Deputy Superintendent of Health Services.

STATEMENT OF RESPONSIBILITIES:
- Maintain Certified Application Counselor (CAC) status with Massachusetts Health
(MassHealth) and serve as the Lead CAC for the facility. - Assist incarcerated individuals with confirmed release dates in applying for, enrolling in, or renewing health benefits through MassHealth.
- Assist in the planning and implementation of the new 1115 Waiver requirements with
Department leadership. - Track offenders under the CAA mandate and collaborate with internal divisions and
partners to ensure compliance. - Coordinate pre-release Medicaid services for eligible individuals in accordance with the Massachusetts Medicaid Section 1115 Demonstration Waiver, which provides coverage for certain medical and behavioral health services during the 90 days prior to release.
- Collaborate with reentry staff, specialty courts, residential treatment programs, and the
Department of Mental Health to address insurance needs and coordinate post-release
health appointments. - Develop comprehensive discharge plans to ensure individuals are prepared for community reintegration, including scheduling medical, behavioral health, and mental health services such as Primary Care Provider (PCP) visits, behavioral health services, and Medication for Opioid Use Disorder (MOUD).
- Establish, maintain, and track relationships with external agencies, healthcare providers, behavioral health organizations, and reentry stakeholders to facilitate referrals and continuity of care.
- Maintain confidentiality and accuracy of all medical records in compliance with federal
and state laws. - Prepare and submit required documentation, reports, and data accurately and on time to the Norfolk County Sheriffs Office (NCSO), Department of Public Health (DPH), and MassHealth.
- Ensure all documentation and practices comply with the standards of the American
Correctional Association (ACA), National Commission on Correctional Health Care
(NCCHC), Massachusetts Depm1ment of Correction (DOC), and Norfolk County Sheriff's Office policies and procedures. - Demonstrate flexibility in work schedule to meet the needs of the facility.
- Perform all other duties assigned by the Sheriff or their designee.
- Must possess a valid Massachusetts Driver’s License
- Must have a minimum of an Associate’s Degree
- Must be a Certified Application Counselor (CAC)
- Minimum 1 year correctional or judicial experience
- Must have a strong understanding of MA Health system
- Proven experience in building partnerships within the community
- Demonstrated ability to coordinate varied number of diverse projects to accomplish established goals
- Must have strong organizational and communication skills
- Position requires frequent communication of data with various criminal justice agencies and departments
